![]() Form GID - 3 appoints any Georgia attorney its attorney-in-fact in the state. ![]() The Insurance Commissioner of Georgia is appointed the legal agent of the companies in Form GID - 4, entitled Appointment of Commissioner as Attorney-in- Fact by Insurer. The bylaws supply much the same information as the charters, but in much more detail. The corporation charters or certificates of incorporation outline the objects, purposes and powers of the companies, indicate names of incorporators and officers and bear the pertinent signatures and notarizations. ![]() The applications of Original Certificates of Authority indicate the location of each company's principal office, the classes of insurance the company proposes to write in Georgia, the form of the company's organization and pertinent dates. All these records are required by the Georgia laws which pertain to licensing and regulating of insurance companies, the chief function of the Insurance Division. Included are: (1) Applications for Original Certificates of Authority (Form GID - 3), (2) corporate charters and amendments, (3) company by-laws and amendments, (4) Powers o f Attorney (Forms GID - 3 and 4), (5) certificates of deposit, (6) verifications of policy forms filings and fees paid, (7) certificates of valuation, (8) biographical data sheets on each company's officers and directors and (9) related correspondence. ![]() This series consists of an archival sample o f charter files of insurance companies conducting business in Georgia.
